Award figures are the CMS FY2026 / Budget Period 1 amounts announced for all 50 states. Five-year figures are a projected run-rate (FY26 award × 5), not a guaranteed total — CMS has stated that future-year amounts may vary with state progress, continuation funding and performance. Status reflects whether a state had at least one RHTP opportunity open at the verification date. Sources: CMS award announcement, state program pages, RHIhub and NRHA trackers. Verified 2026-08-25. Confirm current requirements with your state agency before relying on any figure in an application.

Questions

The ones we get most. Skim the headings.

What is the Rural Health Transformation Program (RHTP)?
The Rural Health Transformation Program (RHTP) is a federal initiative designed to help rural healthcare organizations strengthen workforce capacity, improve clinical quality, modernize technology infrastructure, expand access to care, and build long-term sustainability. Established under Section 71401 of P.L. 119-21, the program represents more than $50 billion in federal investment. CMS announced awards to all 50 states on December 29, 2025, with $10 billion available each year through 2030.
Who qualifies for RHTP funding?
Organizations that may benefit include state health departments, rural health offices, rural health networks, critical access hospitals, rural hospitals and health systems, EMS organizations, home health agencies, nursing schools and academic programs, workforce development organizations, and community-based healthcare organizations. Eligibility and funding structures vary by state and program implementation, so organizations should review state-specific guidance and funding announcements for detailed qualification requirements.
How much RHTP funding is available?
The program represents more than $50 billion in federal investment, with $10 billion distributed each year through 2030. First-year state awards averaged approximately $200 million, ranging from $147 million to $281 million. Half of the funding is distributed equally among all approved states, and half is allocated based on state rurality metrics, rural health system characteristics, state policy actions, and the projected impact of proposed initiatives. Actual funding opportunities and award amounts vary depending on state implementation plans.
How can nursing programs participate in RHTP?
Nursing schools and workforce development programs support rural healthcare transformation through workforce readiness and capacity building. Participation opportunities include: Competency-based nursing education Faculty development initiatives Clinical skills training programs Rural healthcare workforce pipeline programs NCLEX® preparation and readiness programs Continuing education and certification tracking Clinical placement partnerships with rural providers By aligning educational programs with workforce shortages and regional healthcare needs, nursing programs help strengthen the future rural healthcare workforce.
How can home health agencies benefit from RHTP?
Home health agencies face many of the same workforce and coordination challenges as hospitals and rural clinics. RHTP-aligned initiatives can help home health organizations improve staff recruitment and retention, enhance care coordination, strengthen secure communication between providers, reduce administrative burden, improve patient transitions of care, expand workforce training opportunities, and support quality reporting and performance measurement.
What technology investments are eligible under RHTP?
While funding opportunities differ by state, technology investments supporting rural healthcare transformation commonly focus on: HIPAA-secure communication platforms Care coordination solutions Telehealth and virtual care Clinical decision support tools Workforce training platforms Competency management systems Quality reporting infrastructure Workflow automation technologies Secure document and file sharing AI-enabled efficiency tools These investments support the RHTP priorities of innovation, workforce support, sustainability, and expanded access to care.
How can rural hospitals reduce workforce shortages?
Addressing workforce shortages requires a comprehensive strategy focused on recruitment, retention, training, and support. Successful approaches often include accelerated onboarding programs, competency-based training, workforce readiness initiatives, continuing education opportunities, clinical decision support resources, professional development pathways, burnout reduction initiatives, improved communication and collaboration tools, and recruitment partnerships with nursing schools. Context matters: national RN turnover reached 17.6% in 2025, and each percentage point of RN turnover is worth roughly $295,000 per year to the average hospital.
What workforce development strategies align with RHTP?
The Workforce Development pillar focuses on building and sustaining a skilled workforce capable of meeting rural community needs. Key strategies include nursing workforce development, workforce readiness programs, clinical competency management, certification tracking, continuing education, leadership development, recruitment and retention initiatives, allied health workforce training, EMS workforce support, and workforce pipeline partnerships.

Skyscape® Medical Library — evidence-based clinical decision support and point-of-care references. Buzz™ — HIPAA-secure communication, collaboration, file sharing, telehealth support, e-signatures, and care coordination across distributed teams.
Can Skyscape help support RHTP grant planning efforts?
Yes. Skyscape offers grant application alignment support and provides examples of how its solutions align with workforce development, sustainability, innovation, and healthcare access priorities.
